He doesn't remember what his real name is, he can only vaguely remember the nurses from his time in the Asylum calling him "Donnie", but he prefers to be called by Pyramid Head or any other of his many aliases. The clothes he wears were given to him by some of the nicer nurses who didn't beat and abuse him, and he would lose his mind if he lost such clothes.
