He grew up respectively with a good childhood with minor bullying, he never grew attachments to others, neither his brothers nor his parents which led to his scholarship funds going into moving to düsseldorf and attending Medical school there where he ultimately became a successful neurosurgeon. His leading principle being that "all lives are equal" and had a strong desire to save anyone he could, whether it be a young boy over a governor. Despite his workaholic personality, the hospital directors daughter (Eva) pursued him, and ultimately they became engaged till Eva one night brought up an opposing opinion to his belief that all lives are created equal. Later he saves the life of an 11 year old boy who had been shot in the head, over a important political figure which would have boosted his reputation to save, and faces criticism from his coworkers as well as being demoted and his fiancé breaks up with him as well. After visiting the young boy he saved who was comatose, next day hung over he woke up to the police at his house because his three superiors had been poisoned (by the young boy out of thanks to tenma). And then had been promoted to chief of surgery. 9 years later Tenma saves the life of another man who was suspect to murders, and one night runs away from the hospital to which Tenma hurriedly follows till he accidentally witnessed a murder by a blonde man(whomst was the young boy he saved 9 years ago, Johan liebert). Johan revealed he was the one who poisoned his superiors as tenma was now benefiting from being the chief of surgery, then left mysteriously. Tenma panickingly researched Johan and his sister Nina fortner, about them in the newspaper but alas could not find much information till he found Nina herself, who was supposedly 'visting' a friend at Heidelberg castle, afterwards, a man had been killed at the castle after he left with Nina, and consequently her parents. Nina stayed with tenma for a while in hiding, as it came to news tenma was framed for murder because he accidentally left his tie at the crime scene which was recognized my his sour ex fiancé. He ran away, in hiding he learns from an ex-veteran how to shoot a gun. Then devoting his time to finding Johan, the monster he saved, and now out of guilt for doing so he must kill him.)
